摘要:
目的探讨后腹腔镜输尿管上段切开取石术（ RLTP）和微通道经皮肾镜碎石取石术（MPCNL）治疗输尿管上段单发大结石的临床效果，为临床决策提供具体的临床证据。方法 2013年 1月至 2015年 1月高要市人民医院根据入选标准和排除标准入选 57例输尿管上段单发大结石患者，结石直径大于 1.5 cm，随机分为 MPCNL组（ 29例）和 RLTP组（ 28例），比较两组的手术时间、术中出血量、术中中转开放情况、结石清除率、术后胃肠道恢复时间、术后住院时间及尿瘘等并发症情况。结果两组手术均获成功，MPCNL组的手术时间及术后双 J管的留置时间均短于 RLTP组，但出血量 MPCNL组大于 RLTP组，两组在结石取净率、中转开放手术、尿瘘、术后胃肠道恢复时间、尿管留置时间及术后住院时间上差异并无统计学意义。结论 MPCNL和 RLTP处理输尿管上段单发大结石均安全可靠。采用何种手术方式，除了患者的具体情况，还应考虑术者对术式的偏好。
