目的 探讨膀胱颈移动度( BND)对女性压力性尿失禁( SUI)患者的诊断价值。方法 应用经会阴实时三维超声技术,对 95例女性压力性尿失禁( SUI)患者在静息及最大 Valsalva两种状态下的超声图像进行观察并测出膀胱颈移动度( BND),同时与 244例正常女性作对照分析。以临床诊断作为诊断 SUI的金标准,使用受试者工作( ROC)曲线评价 BND诊断 SUI的诊断效能,并确定诊断临界值。结果 最大 Valsalva动作时 SUI组的膀胱颈移动度明显大于对照组,两组结果比较差异有统计学意义( P<0.05)。BND诊断 SUI的曲线下面积为 0.755,最佳诊断临界值为 27.7 mm,BND诊断 SUI的敏感性、特异性、阳性预测值、阴性预测值、准确性分别为 57.89%、80.33%、52.88%、82.98%、73.75%。结论 膀胱颈移动度对 SUI有一定的诊断价值,可为临床提供诊断依据。
Objective To investigate the evaluation of the bladder neck descent (BND) in the diagnosis of female stress urinary incontinence (SUI). Methods Transperineal real-time three-dimensional ultrasound were performed in 244 cases of normal women and 95 cases of female patients with stress urinary incontinence in resting condition and Valsalva condition, the bladder neck descent (BND) were observed and evaluated. The clinical diagnosis were used as the reference standard. And the diagnostic accuracy and diagnostic cut-off value were assessed with receiver operating characteristic (ROC) curve. Results The bladder neck descent on maximum Valsalva action was significantly larger in the SUI group than the control group. The results between the two groups were different significantly (P<0.05). The area under ROC curve of BND for compensated SUI was 0.755, the best cut-off value was 27.7 mm. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of BND were 57.89%, 80.33%, 52.88%, 82.98%, 73.75% respectively. Conclusion s The BND can be applied in the diagnosis of female SUI.
