摘要:
目的评价微通道经皮肾镜（ MPCNL）与输尿管软镜（ fURL）两种手术方式治疗肾结石的有效性和安全性。方法检索 PubMed、EMBase、CBM、CNKI、VIP、万方数据平台从建库到 2015年 1月 1日的文献，对所纳入的研究，运用 RevMan 5.2软件进行 Meta分析。结果共纳入 1篇 RCT研究、7篇病例对照研究，累计样本量为 735例肾结石患者（ MPCNL组 438例，fURL组 297例）。Meta分析表明，MPCNL组在结石清除率和手术时间方面显著性优于 fURL组（ P<0.01）。但对于 <2 cm的肾下盏结石， MPCNL组的结石清除率与 fURL组无统计学差异（ P>0.05）。在术后血红蛋白下降方面， fURL组优于 MPCNL组（ P<0.01）。在术后并发症方面，MPCNL组与 fURL组无统计学差异（P>0.05）。漏斗图提示各文献间未见明显发表偏倚。结论 MPCNL术在结石清除率方面优于 fURL术，但 fURL术手术损伤小、出血少。对于小于 2 cm的肾下盏结石，可以优先选择 fURL术。
