摘要:
目的比较经尿道国产 2 μm激光前列腺光汽化术与经尿道前列腺汽化电切术（ TUVP）治疗前列腺增生（ BPH）的临床疗效及安全性。方法回顾 2011年 6月至 2014年 6月苏州大学附属第二医院泌尿外科收治并手术治疗的前列腺增生患者中 2 μm激光光汽化治疗 94例，电汽化术 156例，共计 250例。记录并比较两组患者术后血红蛋白降低浓度、术后血钠降低浓度、手术前后国际前列腺症状评分（ IPSS）、生活质量评分（ QOL）、最大尿流率（ Qmax）、膀胱残余尿（ PVR）、手术时间、住院时间及术后并发症发生率。结果 2 μm激光组与电汽化组两组术后血红蛋白降低浓度分别为：（4.2±1.4）g/L、（11.3±3.1）g/L，两组间差异有统计学意义（ P<0.05）；两组患者术后血钠降低浓度分别为：（0.9±0.3） mmol/L、（5.1±1.8）mmol/L，两组间差异有统计学意义（ P<0.05）；两组患者术后住院天数分别为：（5.5±1.9）d、（6.6±2.5）d，两组间差异有统计学意义（ P<0.05）；两组患者术后留置导尿管时间分别为：（3.3±0.9）d、（5.2±1.3）d，两组间差异有统计学意义（ P<0.05）。术后两组均未发生电切综合征（TURS）。1个月后及 3个月后随访，两组术后 IPSS、PVR、Qmax和 QOL均较术前明显改善，但两组之间差别无统计学意义（ P>0.05）。结论经尿道选择性国产 2 μm激光汽化术治疗前列腺增生疗效与TUVP手术一样显著，但安全性更高、术后恢复更快，是微创治疗前列腺增生有效的方法。
