摘要:
目的探讨 CT造影在输尿管末端小结石( ≤6 mm)分类治疗方法选择中的价值。方法使用螺旋 CT+三维重建技术,根据输尿管小结石梗阻上方集合系统扩张情况,分为肾盏无扩张组及肾盏扩张组,两组均先采用相同药物排石治疗两周,如排石失败再行输尿管镜钬激光碎石术。结果两组药物排石成功率分别是 94.6%(105/111)、24.4%(10/41)(P<0.05)。两组结石未排出的患者均转入手术组,经输尿管镜碎石取石术治疗,结果所有病例均取石成功。结论螺旋 CT及三维重建为输尿管末端小结石梗阻的分类提供了科学依据,为药物排石及手术方案的合理选择提供依据。肾盏扩张组可选择及早行输尿管镜碎石取石治疗,而肾盏无扩张组患者可选择药物排石等保守治疗,保守治疗失败后再行输尿管镜手术治疗。
