摘要:
目的评价应用国产 160 W绿激光施行功率选择性汽化术治疗大体积前列腺增生的疗效和安全性。方法 2013年 5月至 2015年 3月,苏州大学附属第二医院应用国产 160 W高功率绿激光系统,施行经尿道前列腺汽化术(PVP)治疗前列腺增生(BPH)。本组 BPH患者 37例,平均年龄 71(59~92)岁,前列腺体积平均 116(81~273) ml,国际前列腺症状评分(IPSS)评分平均 27(15~33)分,最大尿流率(Qmax)平均 6.0(3.6~9.3)ml/s,膀胱残余尿(RUV)平均 252(18~1432)ml。比较患者手术前后血红蛋白变化、记录手术时间、术后留置导尿管时间、术后住院时间以及手术后并发症的情况。术后 3个月随访 IPSS、生活质量评分(QOL)、Qmax的变化。结果本组手术时间平均 96(35~170)min,激光耗能平均 520(345~1082)kJ,所有手术均一次完成,术后留置导尿管 1~6 d,平均 3d。32例患者术后完成 3个月周期的随访,IPSS由术前 27分下降至术后 3个月 8(3~18)分(P<0.01); Qmax由术前 6(4~9)ml/s上升至术后 3个月 15(13~18)ml/s(P<0.01); QOL由术前 4(3~6)分下降至术后 3个月 2(1~4)分(P<0.01)。结论应用国产 160 W绿激光施行 PVP治疗大体积 BPH同样安全、有效、恢复快,是目前治疗大体积 BPH理想的微创治疗方法。
