摘要:
目的 采用Cochrane系统评价方法对经皮肾镜取石术与开放手术治疗肾结石的临床有效性、安全性进行评价.方法 按Cochrane系统评价方法,计算机检索Cochrane library、Medline、EMbase、中国期刊全文数据库、生物医学文献数据库及维普数据库;手工检索中文泌尿外科期刊的相关文献.收集经皮.肾镜取石术,对比开放手术取石治疗.肾结石的随机和非随机对照试验.质量评价后,采用RevMan4.2版软件进行Meta分析.结果 纳入1个随机对照试验和6个非随机对照试验,共计1459例.经皮.肾镜取石术和开放手术取石术在结石清除率[OR=1.31,95%CI:(0.96,1.77)]、术后出血[OR=I.31,95%CI:(O.96,1.77)]没有差别,但经皮.肾镜取石术的切口感染[OR:0.03,95%CI(O.00,0.21)]、邻近器官损伤[OR=0.18,95%CI(O.08,037)]及住院时间[WMD=一2.70,95%CI(一2.80,一2.61)]等明显低于开放手术取石术.但败血症发生[OR:2.52,95%CI(1.34,4.75)]方面,开放手术取石术低于经皮.肾镜取石术.结论 在.肾结石的结石清除率、术后出血方面,经皮.肾镜取石术与开放手术取石术无差别,而在切口感染、邻近器官损伤、住院时间方面,经皮.肾镜取石术明显少于开放手术取石术,但经皮.肾镜取石术的败血症发生多于开放手术.
